Non-UND Occupational Health Forms
All IUSM Staff or other non-affiliated personnel will need to submit paper Category A or Category B forms for the Occupational Health program to U.S. Healthworks. Non-UND personnel include individuals employed by Indiana University, outside contractors or collaborators from other institutions. The Category A or B Occupational Health forms can be accessed through the FLSC website at http://freimann.nd.edu/training/occupational-health/. You will have to mail the forms to U.S. Healthworks at 19567 Cleveland Road, South Bend, IN 46637. All Notre Dame personnel (students, faculty and staff) are required to submit Category A or B forms online through the Onbase system. Any UND personnel who submit paper forms will have to pay for the medical review at U.S. Healthworks AND resubmit through Onbase at an additional charge.
Lab Animal Technician Week
International Laboratory Technician Week is Jan. 28-Feb. 3. This is an opportunity for researchers to say “Thank you” for the invaluable assistance the FLSC technicians provide.
Animal Orders and Procedure Requests
FLSC has gone paperless for animal orders and animal procedure requests. All animal orders and procedure requests will now be placed through the CORES system. This change is in keeping with the University’s effort to increase efficiency, simplify billing activity and maintain better documentation of services. Please contact Gwen Stayback at 1 6085 if you have any questions regarding the use of the system.
IACUC Meeting Schedule
IACUC meetings for the 2018 Spring semester will be held Wednesdays from 9:30-11:30 a.m. The meeting dates scheduled are Jan. 17, Feb. 14, Mar. 14, Apr. 11, May 16, June 13 and July 11. Protocols should be submitted no later than 10 days before a scheduled meeting date.
All compounds brought into the animal facility must have a Safety Data Sheet (SDS) on file with the Animal Facility.
This includes drugs, reagents and chemicals that will be used in animals. Attach them to the IACUC Animal Use Protocol and provide a printed copy to Val Schroeder at FLSC.
